This is a new service available through Roots and Branches (R&B) to convert 4.0+ software
NQs back to the clinically validated 3.0 software at the core of Dr. Shoemaker’s research. The
service is $195 per scan and includes the General Morphometry Report and the Age-
Related Atrophy/Dementia Report. Each additional report is an extra $50 as it requires
rerunning the scan. If the MRI is not run with the proper specifications as outlined below
and the NQ processing fails, $100 of the $195 is non-refundable.
This is a cash pay service and is not billable to insurance.
Please follow the following instructions to take advantage of this service.
Step 1: Have your patients get a Brain MRI without contrast in 3D with a T-1 Sagittal
Sequence protocol. The scan MUST be run with this sequence so make sure this information is
present on the original referral. See page 2 of this document for additional information.
– Step 2: Have your patient sign a HIPAA release form giving us permission process their
imaging and the associated reports.
– Step 3: Send a CD of the scan in DICOM format to the following address (183 S. Taylor Ave
Set 158, Louisville, CO 80027) and include both the HIPAA Release and the following
Conversion Request Form. Please note, you will not receive the CD back!
– Step: 4: Once R&B receives the CD and related forms, we will process payment for the scan
(in the manner indicated on the request form) and proceed with the conversion. Once the
reports have been generated, we will send them back to you in the format specified on the
request form.
